Please read the following information. It summarizes SOGIP’s visitor HSE requirements.

SOGIP cares about your safety, health, and the natural environment. You also have responsibility to care for your own and other’s health and safety.

After reading this information you will be required to sign this advice to acknowledge your understanding and cooperation with SOGIP’s visitor safety requirements. Your site visit specific requirements will be advise during safety induction.

BASIC SAFETY RULES

It is necessary that all employees, contractors, visitors, vendors, and others third parties must be fully aware of some basic safety requirements to be followed.

GENERAL HSE REQUIREMENT

• Wear appropriate clothing when entering premise/workplace area (no singlet, no shorts, no sandals, etc.)
• Please comply with all safety signs.
• If an emergency occurs during your visit, you must follow instruction from person in charge.
• Please do not create hazards that may cause injury to yourself and others.
• Please report all hazards and incidents (accident and near-miss accident) to person in charge or site manager.
• Reverse Park your vehicle & obey the road speed limits while driving.
• Smoking only at designated smoking area.
• No unauthorized photo taking and do not enter any area without authorization.
• No separation, visitor will be walking in a coordinated group around site.
• All newcomers are to report to HSE for the safety induction after all forms are complete.
